The short answer: In trials, retatrutide lowered blood sugar and body weight within the first 12 weeks. By week 24 of the phase 2 obesity trial, the 12 mg group had lost 17.5% of body weight on average. Weight kept falling after that, to 24.2% at 48 weeks. In the phase 3 TRIUMPH-1 trial, the 12 mg group averaged a 25.0% loss at 80 weeks counting everyone randomized, or 28.3% among people who stayed on treatment. Retatrutide is investigational and not FDA approved, so these are trial results, not a promise for any one person.
How soon after the first dose does retatrutide act?
The earliest data come from a 12-week phase 1b trial in 72 adults with type 2 diabetes, published in The Lancet in 2022. Retatrutide was still called LY3437943 at the time. Participants took one injection a week.
By week 12, the three highest dose groups had clear drops in blood sugar compared with placebo. Their placebo-adjusted HbA1c fell by 1.2 to 1.6 percentage points. Body weight also fell, by up to 8.96 kg (about 20 lbs) more than placebo in the group that stepped up to 12 mg.
The same trial found a half-life of about 6 days. That means a weekly shot builds up over several weeks before levels steady out. Our post on how long retatrutide stays in your system explains the half-life math.
No published abstract we read reports weight change after the first week or first month alone. So any claim about results in "week one" is not backed by a published trial figure.
How much weight is lost by week 24?
The main early benchmark is the 24-week mark in the phase 2 obesity trial. Jastreboff and colleagues enrolled 338 adults with obesity, or overweight plus a weight-related condition. The primary end point was weight change at 24 weeks.
Mean weight change at 24 weeks was:
- 1 mg: -7.2%
- 4 mg (combined arms): -12.9%
- 8 mg (combined arms): -17.3%
- 12 mg: -17.5%
- Placebo: -1.6%
The trial's ClinicalTrials.gov results record shows how many people had crossed common thresholds by week 24. In the 12 mg arm, 97% had lost at least 5%, 90% had lost at least 10%, and 70% had lost at least 15%. On placebo, the figures were 26%, 3%, and 1%.
Those numbers came about halfway through the trial. Our guide to how much weight people lose on retatrutide covers the final results dose by dose.
Why does the full effect take more than a year?
Two things slow the curve: dose escalation and time. Trials do not start people on the top dose. They start low and step up.
In the phase 3 TRIUMPH-1 trial, everyone on retatrutide started at 2 mg once weekly, per Lilly's May 21, 2026 announcement. The dose rose every four weeks. The 4 mg group took one step. The 9 mg group stepped through 2, 4, and 6 mg. The 12 mg group stepped through 2, 4, 6, and 9 mg.
By that schedule, the 12 mg group spent its first 16 weeks on lower doses. That figure is our arithmetic from the four 4-week steps, not a number Lilly published.
Weight loss then kept building well past that point. In the phase 2 trial, the 12 mg group went from -17.5% at 24 weeks to -24.2% at 48 weeks. In TRIUMPH-1, Lilly reported these average losses at 80 weeks for people who stayed on treatment (the efficacy estimand):
- 4 mg: -19.0% (47.2 lbs)
- 9 mg: -25.9% (64.4 lbs)
- 12 mg: -28.3% (70.3 lbs)
- Placebo: -2.2% (5.5 lbs)
The NEJM paper on TRIUMPH-1 counted everyone randomized, including people who stopped the drug (the treatment-regimen estimand). On that basis, the 80-week averages were -17.6% (4 mg), -23.7% (9 mg), and -25.0% (12 mg), versus -3.9% on placebo.
An extension in people who started with a BMI of 35 or higher ran to 104 weeks. Those who stayed on 12 mg averaged a 30.3% loss. So the drug works early, but most people in trials did not reach their lowest weight for well over a year.
The doses and step schedules from each trial are listed in our post on retatrutide trial doses.
When does appetite change?
Retatrutide acts on three hormone receptors: GIP, GLP-1, and glucagon. GLP-1 drugs as a class reduce appetite, and the glp1.md site covers how that class works.
For retatrutide itself, the best appetite data come from the phase 2 type 2 diabetes trial. A 2025 analysis in Diabetes, Obesity and Metabolism used appetite questionnaires in 275 adults. Compared with placebo, people on 4 mg or more reported less overall appetite, less hunger, and lower expected food intake at week 24.
At 8 mg and 12 mg, people also reported less perceived hunger and less tendency to overeat at weeks 24 and 36. Larger drops in hunger and overeating went with larger weight loss by week 36.
That analysis reported results at weeks 24 and 36. It does not tell us how many days after the first shot hunger starts to fade.
How fast does blood sugar fall?
Blood sugar responds faster than weight. The phase 1b trial already showed HbA1c drops at 12 weeks, as noted above.
The larger phase 2 diabetes trial, by Rosenstock and colleagues in The Lancet, enrolled 281 adults with type 2 diabetes. Its primary end point was HbA1c change at 24 weeks. The 12 mg group fell 2.02 percentage points, and the 8 mg slow-escalation group fell 1.99. Placebo fell 0.01, and dulaglutide 1.5 mg fell 1.41.
The authors wrote that the findings were consistent at 36 weeks. Weight fell 16.94% at 36 weeks on 12 mg, versus 3.00% on placebo.
Does a faster start mean faster results?
Slightly, at first. The phase 2 obesity trial tested two starting doses for the 4 mg and 8 mg arms. The registry results show the arms that started at 4 mg had lost a bit more by week 24:
- 4 mg, started at 2 mg: -12.00%
- 4 mg, started at 4 mg: -13.80%
- 8 mg, started at 2 mg: -16.64%
- 8 mg, started at 4 mg: -18.26%
The trade-off was side effects. The NEJM abstract says stomach and bowel side effects were dose related and were "partially mitigated with a lower starting dose (2 mg vs. 4 mg)." That is one reason the phase 3 program starts everyone at 2 mg.
Heart rate also rose with dose. In the phase 2 obesity trial, that rise peaked at 24 weeks and declined after that.
The bottom line
Retatrutide showed measurable effects on blood sugar and weight within 12 weeks in early trials. By 24 weeks, most people on 8 mg or 12 mg had lost at least 10% of their weight. The largest losses took 48 to 104 weeks, partly because doses are raised in 4-week steps. Retatrutide is investigational and is legally available only to participants in Lilly's clinical trials.
