The short answer: No published retatrutide trial has reported hair loss as a common side effect, but the data are thin. The phase 2 obesity trial did not list hair loss among events seen in 5% or more of any group, and Lilly's phase 3 releases do not mention it. Semaglutide and tirzepatide, which cause similar weight loss, both list hair loss on their FDA labels at about 3% to 6%. Retatrutide is not FDA approved, so there is no label yet to settle the question.
Did retatrutide trials report hair loss?
Not in any data published so far. The phase 2 obesity trial randomized 338 adults to retatrutide or placebo for 48 weeks. Its ClinicalTrials.gov results record lists non-serious adverse events that occurred in at least 5% of any treatment group. Alopecia and hair loss are not on that list.
That is not the same as zero. An event seen in 3% or 4% of people would fall below the reporting cutoff and would not appear. The skin events that did clear the 5% line were small: rash, pruritus and "sensitive skin," each in two or fewer people per group.
The phase 3 program is larger. Lilly's TRIUMPH-1 release (21 May 2026) lists nausea, diarrhea, constipation, vomiting and dysesthesia. It does not name hair loss as a retatrutide event. The phrase "hair loss" does appear in that release, but only in the safety information for Zepbound and Foundayo, two approved Lilly drugs. The TRIUMPH-4 release (11 December 2025) also says nothing about hair.

How often do Wegovy and Zepbound cause hair loss?
These two drugs are the closest comparison, and both FDA labels list hair loss. The numbers below come straight from the current labels on DailyMed.
- Zepbound (tirzepatide). In the pooled weight loss trials, hair loss occurred in 5% on 5 mg, 4% on 10 mg and 5% on 15 mg, against 1% on placebo. The Zepbound label says it was reported in 7.1% of women and 0.5% of men on the drug. No patient on Zepbound stopped treatment because of it.
- Wegovy (semaglutide). The Wegovy label reports hair loss in 3.3% on 2.4 mg versus 1% on placebo. In trials of the 7.2 mg dose, the rate was 5.8% (8.4% of women, 0.2% of men).
Both labels use the same sentence: hair loss "was associated with weight reduction." That phrasing points to the weight loss itself, not to a direct effect of the drug on hair. The tirzepatide story is covered in more depth at mounjaro.md.
Why does fast weight loss thin hair?
The usual cause is telogen effluvium. Each scalp hair cycles between a growing phase (anagen) and a resting phase (telogen). A shock to the body can push many growing hairs into the resting phase at once.
A review in Clinical, Cosmetic and Investigational Dermatology describes the pattern. Resting hairs stay in place for about 3 months, then fall out together. The classic description puts the shedding 3 to 4 months after the trigger. Acute cases are self-limited. The review calls the condition chronic only when it lasts beyond 6 months.
The same review names severe diet and iron deficiency as claimed triggers. Large, fast drops in calorie intake fit that description. So do the losses seen in obesity drug trials. What do dermatology studies say about incretin drugs and hair?
Three 2026 papers pulled the evidence together. None of them included retatrutide data.
- A meta-analysis in Diabetes Research and Clinical Practice pooled nine interventional studies with 4,114 GLP-1 drug users. Hair loss was about 3.3 times more likely than on placebo. The overall event rate was 3.9%.
- A systematic review in Science Progress found the highest rates with semaglutide and tirzepatide. Telogen effluvium and androgenetic alopecia were the main types. Women were affected more often, and rapid weight loss was a likely contributor.
- A commentary in Dermatology and Therapy noted that no prospective controlled study has looked at this question directly. The authors wrote that causality "has not been established."
Is retatrutide likely to be different?
Nobody knows yet, and no one should claim otherwise. Retatrutide acts on GIP, GLP-1 and glucagon receptors. If hair loss is mainly a side effect of rapid, large weight loss, a drug that causes more weight loss could plausibly cause as much or more shedding. That is a reasonable hypothesis, not a finding.
What the trials do show is a different skin signal. Dysesthesia (an altered or painful sense of touch on the skin) occurred in 5.1%, 12.3% and 12.5% of people on 4, 9 and 12 mg in TRIUMPH-1, versus 0.9% on placebo. In TRIUMPH-4 it reached 20.9% on 12 mg. That is a skin sensation, not hair loss.
Anyone taking "research" retatrutide bought online faces extra unknowns: dose, purity and what is really in the vial. Read whether gray-market retatrutide is safe before you consider it.
What can you do if your hair starts shedding?
Talk to the doctor who prescribes your medicine, or to a dermatologist. Diffuse shedding has several causes, and a proper exam sorts them out.
- Check the timing. Shedding that starts a few months after rapid weight loss fits telogen effluvium, which usually settles on its own.
- Ask about labs. A clinician may check iron stores, thyroid function or other levels based on your history.
- Avoid extreme restriction. The dermatology review lists severe diet as a claimed trigger, so ask about a realistic eating plan.
- Be wary of pills. The Rebora review states the literature does not support biotin or vitamin E supplements for this condition. Excess vitamin A can make hair loss worse.
Do not stop or change a prescribed drug on your own. Your prescriber can weigh the shedding against the benefit of the treatment.
The bottom line
Published retatrutide data do not list hair loss as a common side effect. The phase 2 record shows no alopecia at the 5% reporting threshold, and phase 3 press releases do not mention it. Wegovy and Zepbound labels list hair loss in 3% to 6% of users, mostly women, and tie it to weight reduction. Retatrutide produced a mean 28.3% weight loss on 12 mg, so some shedding would not be surprising. Full phase 3 papers should give a real answer. Retatrutide is investigational and not FDA approved.
